New Patients

New Patient Registrations

We are currently accepting New Patients wishing to register within our catchment area. (Please check the Practice Area below)

All newly registered patients are asked to complete a questionnaire by clicking the link below. It asks for details of your health, family history and current medication.

It is important to provide us with this information as it provides the doctor with information about you whilst we are waiting to receive your notes from your last practice.

To prevent delays with the registration process and moving practices a form of personal ID such as a driving license or passport, and proof of your current address may be required along with the completed questionnaire, but this will only be required if there is an issue/query with your registration. (We will contact you and ask for this info if needed)

 A Birth Certificate is required for new babies and children.

Please note proof of address is not mandatory to register for GP services and if you are homeless, sofa surfing, a refugee, or for any other reason you cannot provide this information your application to join the practice will not be refused. 

In exceptional circumastances we may accept Out of Area registrations on the understanding that we will not be able to visit you at home.

Temporary Patient Registrations

If you become unwell while away from home or if you are not registered with a Doctor but need to see one you can receive emergency treatment from the local GP practice for up to 14 days.  If after 14 days you require further treatment you will need to register as a temporary or a permanent patient.

You can be registered as a temporary patient for up to three months.  This will allow you to be on the local practice list and still remain a patient of your permanent GP.  After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use.  P actices do not have to accept you as a Temporary patient although they do have an obligation to offer emergency treatment.  You cannot register as a Temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities

This is available through the main front door. If you have difficulty getting to the front door from the car park we can arrange for you to come in through the back door. Please ask a receptionist about this when making your appointment.

The disabilty toilet is located on the ground floor close to the front entrance.
