Patients need to Register for the online 
services with the practice.  It is a simple
process but we do need photoID e.g. 
Driving Licence with your current address or passport
GP to GP transfer (electronic transfer 
of computerised medical records between the patients registered GP)
GP to GP transfer (electronic
transfer of computerised medical recordsbetween the patients registered GP)
Electronic records within SystmOne* 
are automatically transferred to thepatients registered GP if on SystmOne. Other transfers are undertaken by GP toGP  

* System One is one of the medical records software supplied to General Practice Nationally

Summary Care Record

 (medications, allergies and adversereactions)

Achieved, all our patients have a Summary Care Record created unless anopt out form is received.Summary Care records can be accessed by GP’s, out of hours, A&E whenpatients attend, to facilitate safer prescribing outside the practice.

Patient Access to Medical Records (medications,allergies and adverse reactions)

Unavailable, under local/national IT services control, no roll our dateas yet.Patients will have access to part of their medical records by 2015.

Electronic Prescriptions

(electronictransfer of prescriptions to pharmacies)
Not Achieved but we have the software to do it.  We expect to enable this facility later inthe year

Will not be suitable for allpatients/medications.   Patients will need to nominate a pharmacy.
