We are currently accepting new patients wishing to register within our catchment area, which can be checked with a receptionist. Please note, we do not cover Wootton Fields or Grange Park.
All newly registered patients will be asked to collect and complete a questionnaire with details of your health, family history and current medication. It is important to us as it provides the doctor with information whilst we are waiting to receive your notes from your last practice. A form of personal ID such as a driving license or passport, and proof of your current address should be brought along with the completed questionnaire. This is to prevent delays with registration and moving practices. A birth certificate is required for new babies and children.
On your first visit to see your doctor, it is useful to take a list of your current medication. The list on the right hand side of your last prescription is ideal as it specifies amount and dosage.
We do not offer this service at present but hope to do so in the future.
