All newly registered patients will be asked to make an appointment with our healthcare assistant for a health check. You will also be asked to complete a questionnaire with details of your health, family history and current medication. This is important to us as it provides the doctor with information whilst we are waiting to receive your notes from your last practice. On your first visit to see your doctor, it is useful to take a list of your current medication. The list on the right hand side of your last prescription is ideal as it specifies amount and dosage.
